POLICE RECORD CHECK FORM
You are required to bring a current Police Record Check with vulnerable sector screening to your placement on Monday April 28th. The check should have been issued within the last 6 months.
NOTE: For those of you will be observing at Homestead P.S., you need to submit your current record check to the Peel Board office in a sealed envelope marked confidential. Include your full name and nature of duties (York U observation visit), and the words “Criminal Record Check” on the envelope. Original document must have been completed by a police service within the preceding 6 months. Submit your check to:
